- Please return your Application For Pro-Am Entry and Contestant Information Form to the Tournament Office as soon as possible either by fax (516) 224-6378 or online. All Pro-Am contestants will be utilizing their full handicap with a maximum of 27. If you belong to a club, they can provide you with your personal GHIN number. This number, via the GHIN system, will provide us with your most recent handicap.
- If you do not have a GHIN number you are requested to provide copies of five recent scorecards signed by the corresponding club pro.
Once again, we welcome you to Long Island’s premier sporting event. We have a great day of championship golf planned. Additional details will be forthcoming.
